If You may receive a 1095-C tax form via US mail.  This is a new tax form that is required to be sent to all employees by March 31, 2016, as a result of the Affordable Care Act.  The 1095-C is for Self-Insured Plans and shows you qualified for health coverage (minimum essential coverage) for some or all months during 2015.  The form requires that we show the Employee Share of the Cost of Employee Only Coverage.  If you have filed your tax return already, there is no need to file an amended return.  The information you needed to file your taxes was on your W-2.
